- Create an individualized plan of care focused on the patient’s specific needs to ensure comfort at all times and an end-of-life service with dignity and respect.
- Assess and manage all symptoms related to the illness such as pain, nausea, anxiety, fatigue, or shortness of breath, etc.
- Address emotional, spiritual, and social aspects of coping with serious illness, honoring specific customs, traditions, and religious practices.
- Help the patient and family navigate through the complexities of end-of-life decision-making, providing resources on matters, such as advance directives, do-not-resuscitate orders (OOH-DNR), or offer community resources available for assistance.
- Provide bereavement support for family members for one year following the patient’s death.
